Care Request Form

Our Care Team consists of elders, church staff and lay leaders. We seek to support you by listening, prayer, providing resources and meeting your needs when necessary.

                                    We love you and want to support you.

We offer three categories of care to members of Fellowship Memphis; Home, Health and Hope .

Home: Includes but is not limited to the following: pre-marital counseling, marital counseling, family counseling, childcare, birth of a child, foster and adoptive parenting, single parents, death of spouse or child, miscarriage, safety for spouse or child, and financial guidance for home expenses.

Health: Includes but is not limited to the following: hospitalization, resources for addressing mental health concerns, and assistance for families suffering loss due to medical issues.
 
Hope: Includes but is not limited to the following: one-on-one meetings, support groups, grief support, and funeral planning.
